Introduction

The ageing population, the increased openness in talking about end-of-life care decisions and the expansion of palliative care eligibility from cancer patients to people touched by other types of life threatening illnesses are all reasons for the raising local and global concern over the provision of end-of-life care in the community. In response to this trend, The Hong Kong Jockey Club Charities Trust initiates the Jockey Club End-of-Life Community Care Project in 2015, supporting the implementation of 5 innovative models in end-of-life community care.

The international conference 2018, titled Innovation and Impact: The Review and Vision of Community End-of-Life Care, hopes to provide a platform for local and international practitioners, educators and researchers exchanging inspiring practice and research wisdom as well as establishing partnership. With the four renowned keynote speakers from four different continents as well as speakers from local and the region, we envision the flourishing of effective, innovative and sustainable service models in the social and healthcare sectors. Specifically, participants would be able to

  • Grasp the updated knowledge on the development and impactful innovative practices of holistic end-of-life care both locally and internationally;
  • Learn about the latest information on assessment of impact of community end-of-life care practices;
  • Refine their ideas in quality end-of-life care though exchanges and networking
